Genitourinary syndrome of menopause (GSM) is the result of declining estrogen. The mucous membrane thins, blood flow decreases, pH rises — and the outcome is dryness, painful intercourse, more frequent infections, and an overall drop in libido. This is a biological problem that, without effective treatment, quickly becomes a relationship problem too. GSM often leads to relationship tension: pain leads to avoidance, avoidance to misunderstanding, and misunderstanding to strain between partners. Without an explanation, a partner often reads the change as a loss of interest.

What actually works

The foundation: Local estrogen

Cream, tablets, or a vaginal ring directly restore the quality of the mucous membrane. The result is better hydration, improved blood flow, and a lower pH, which significantly reduces the frequency of infections and urinary tract inflammation. Without this treatment, the tissue does not return to its original state on its own.

Supportive care

  • Lubrication and hydration have their place. A lubricant during intercourse (ideally a combination of water- and oil-based products) should be the standard, not an emergency fix.
  • Regular use of hydrating gels helps maintain comfort between intimate moments.
  • Hyaluronic acid–based gels (they bind water) help with dryness.
  • Microflora support — lactobacilli in the form of gynaecological probiotics.

Communication and intimacy

This is where it’s decided whether the problem deepens or stabilises. What works:

  • Name the issue directly — it’s a physical condition, not disinterest.
  • Reset expectations — more time, less pressure.
  • Broaden intimacy — a wider spectrum of closeness, not only intercourse.

GSM is a common condition, yet it often remains taboo. Without treatment it typically doesn’t improve — on the contrary, it tends to gradually get worse. That’s why “just adapting” or relying on lubrication alone isn’t a long-term solution. The key is a combination of local estrogen, supportive care for comfort, and open communication, which prevents the physical issue from spilling into the relationship. Only these steps together restore not just physical comfort, but also natural intimacy — without pressure or misunderstanding.
